Job Summary

Community Health Care Workers (CHWs) work in the neighborhoods where patients reside connecting and linking patients and their families to resources and the care they need. Serves as a member of the care model team enabling population health in the community by building trust and vital relationships with residents of assigned medical neighborhood. CHWs will complete CHW Training and Certification after hire.

Accountabilities

  • Develops strategic partnerships in the community to support community outreach and referrals. \u00A0Establishes peer-to-peer relationship in order to function as a link between the community and the health system. Provides guidance and support for patients through home visits and support of care coordination programs.
  • Conducts weekly home visits to CHW clients.\u00A0\u00A0 Assists individuals within their community in managing their own health including promoting compliance with appointments, screenings, treatment and medications. Assists patients in navigating the health care system.\u00A0 Facilitates assistance or interventions for patients between care visits.
  • Be an advocate for patients by staying current of community resources.\u00A0 Provides resources to patients to improve their health knowledge and self-sufficiency, help them understand their health condition(s) and develop strategies to improve their health and well being.\u00A0
  • Serve as a liaison between patients and clinical care team.\u2002Reports back to their clinical liaison on encounters and the results of such encounters.
  • Coordinates meetings for CHW's to communicate and share best practices. Coordinates meetings with Clinical liaison.

Supervisory/Management Responsibility

This is a non-management job that will report to a supervisor, manager, director, or executive.

Minimum Requirements

  • High School diploma or equivalent OR post-high school diploma / highest degree earned
  • 1 year of community involvement experience.\u00A0

Other Required Skills and Experience

  • Some positions may require that incumbents be bi-lingual in Spanish and English
