Job Summary

Based in the Prisma Health Richland Level 1 Adult and Level 2 Pediatric trauma center, a newly established hospital-based violence intervention program (HVIP) aims to prevent and reduce community and interpersonal violence among high-risk youth and adults. This role provides wrap-around, multidisciplinary services to address underlying risks and social determinants of violence. The role also provides direct patient services including performing needs assessments, mentorship, case management, coordination with community services, and long-term patient and family support.

Accountabilities

*Provide support and violence retaliation intervention utilizing peer mentorship and trauma-informed care principles.

*Perform patient risk and needs assessments, develop action plans with patients, and coordinate multi-disciplinary hospital and community services to help address individuals\u2019 risks, social determinants of health and recovery utilizing health care and community resources. These may include identifying and connecting patients to services pertaining to health and mental health, substance abuse, housing, employment, education, healthy relationships, criminal justice advocacy, community arts and recreation programs, and other resources to address risks, needs and goals.

*Provide long-term follow-up and case management to enrolled patients along with inpatient and outpatient care coordination including in person home and community visits.

*Provide hospital and community outreach and education to engage and educate healthcare trainees, hospital staff, community members and stakeholders on violence prevention and intervention strategies, and trauma informed care.

Supervisory/Management Responsibility

This is a non-management job that will report to a supervisor, manager, director, or executive.

Minimum Requirements

High School diploma or equivalent.

Bachelors degree preferred.

2 years of community involvement experience.\u00A0

Must have reliable transportation, valid driver\u2019s license, and a certificate of auto insurance.

Other Knowledge, Skills and Abilities

Candidates with personal knowledge and experience in overcoming violence/ violence-related injuries/ the criminal justice system preferred.

Knowledge of local community issues, specifically community and youth violence preferred.
